Re-Ablement/Rehab.

Stuck in bed after a fall? Have you had surgery or a hospital stay and lost skills you can’t seem to regain?

We can get you back on your feet, with a limited time rehab package that aims to regain lost skills. We offer Re-Ablement/Rehab Assessments to gage your needs and provide an exercise program or equipment provision.

Slow the decline at the Pre-Diagnosis or early stages of Dementia with the help of Occupational Therapy, with 6 hours of therapeutic work and ongoing review to assess needs within 2-3 months.

But what exactly is Occupational Therapy?

What is Occupational Therapy?

Occupational Therapy is a function in healthcare that you might not be familiar with. It is based on improving engagement in meaningful activities of daily life. This could include self-care skills, education, work, or social interaction. It’s especially used to enable or encourage participation in such activities despite impairments or limitations in physical or mental functioning.

What is an Occupational Therapist (OT)?

Occupational Therapist’s are health care professionals who provides support, based on evidence, to people whose health prevents them from being independent doing activities that matter to them. These are often called “activities of daily living.”

What are “activities of daily living?”

An activity of daily living, is anything you choose to do to fill your time. From washing and dressing to making a meal or even going to the shops. 

What does our Occupational Therapist offer you?

Our Occupational Therapist’s provide free initial assessments to see what your strengths are and how we can work on them to improve your independence. Every six months you will also be provided a free OT assessment to review your progress. This is half the time frame recommended by the CQC or completed by Social Care, but we think it’s worth it to ensure you are getting the very best care fo you and your goals.  

What are specialist assessments?

At your free initial assessment, if you or your Occupational Therapist identify any specific areas of concern, such as difficulty remembering things or not being able to sit out in your chair, a special assessment designed around that particular area will be completed. This gives us a better understand of your condition and how we can best support you, either with at home exercise programs, or equipment or maybe referrals to other health care professionals.  

From your Initial assessment to your support care plan based on specialist individual assessment and reviews every 6 months, you know you're in the best hands. This is all free and included in your service.
